Can ceramic-on-ceramic bearings help to improve the THA outcomes in teenaged patients? The treatment of pediatric hips is often complex and requires special attention. In a recent retrospective clinical study, P. Buddhev et al. (Stanmore, UK) reviewed the indications and mid-term outcomes of 60 THA with ceramic-on-ceramic bearing couples performed by one single high volume senior surgeon on adolescents (35 female patients, 16 males; mean age 16.7 years, range 12-19) over the time period 2003-2017.
After a mean follow-up of 9.3 years, the results show a 97% survival rate. Two patients had to be revised for late-onset aseptic loosening. One of them developed squeaking as a consequence of the loosened acetabular cup. Both patients have been converted to THA from arthrodesis, which is associated with high complication rates.

The authors concluded that due to the complex anatomy of teenagers, such cases require not only thorough pre-op planning, but also a careful implant selection and are “best managed” with CoC bearings.
